You may request your child’s transcripts and other student records by visiting the Student Wellness & Support Services Office at 645 Wool Creek Drive, San Jose, CA 95112. The office is open from 8:00 a.m. to 4:30 p.m., Monday through Friday.

The following records may be requested in person:

• Transcripts/Immunizations for current students and alumni

• Daily Attendance Records

• Proof of Enrollment

• Grades

• Behavior Summary

We cannot release student information over the phone. In order to receive student information, you must make this request in person at the Student Wellness & Support Services. Please bring a valid ID with you (i.e. Driver’s License).
